In January 2013, a request was sent out through the Summit Group email listserv for the most current case file review tool or instrument from each state Vocational Rehabilitation agency. A third of all state Vocational Rehabilitation agencies sent a response. Those that responded sent their instrument and several sent the instructions that are given on how to complete the instrument.
