(9/25/2018) We are interested in exploring the methods other states are utilizing to complete the Semi annual and Annual reviews of individuals in Extended Employment to be in compliance with the regulations regarding obtaining signatures from the individuals on the reviews. Are you completing the reviews and obtaining the signatures in person, electronically, by mail or any other method? What are the response rates? What system are you using to keep track of the reviews? We are considering the option of mailing out the surveys. Before we implement the process, we would like to get feedback from other states on the processes they have in place, what works and what doesn’t.

Our staff does on site presentations to 14C employees and obtains signatures from the consumers on the form below. The employers are given these forms. Several of the employers have had visits by Dept. of Labor and they were pleased by the documentation. Additionally, at the request of RSA, we created a database in which we enter the consumers name, date of employment, age, date we completed the required activities, and when we need to return. Keep in mind we only have 9 known 14C employers and around 500 employees.
